What Is Group Adult Foster Care (GAFC) in Massachusetts?

Group Adult Foster Care in Massachusetts is a MassHealth program that connects adults who need daily assistance with trained caregivers in a structured, supportive setting. It supports adults 22 and older, including people living with physical disabilities, cognitive impairments, or chronic conditions that affect their ability to manage daily life independently.

The program works through three connected pieces. MassHealth funds it. Licensed GAFC providers coordinate and deliver the care. And MassHealth-approved assisted living residences or group housing settings provide the stable, supervised environment where that care happens. GAFC does not cover room and board. It covers the care delivered within that setting.

What Services Does GAFC Cover?

GAFC covers two categories of daily support:

Activities of Daily Living (ADLs) : Bathing, dressing, grooming, eating, mobility, and toileting.

Instrumental Activities of Daily Living (IADLs):  Meal preparation, housekeeping, laundry, transportation to appointments, and medication management.

A trained direct care aide provides one hour of hands-on support each day inside the member’s approved setting. The program also includes nursing oversight and case management, with a registered nurse and care manager conducting regular visits to review each member’s care plan, monitor health, and adjust support as needs evolve.

Step 4: Admission and Start of Care

Once approved, a registered nurse and case manager conduct an admission visit, your care plan is established, and daily services begin.

What is the difference between GAFC and AFC?

GAFC supports adults living in approved assisted living residences or group housing settings, where a direct care aide provides hands-on support daily. AFC is a live-in program where a caregiver, often a family member, lives with the person receiving care in a private home and receives a monthly stipend. The two programs are distinct and cannot be combined.

Does income affect GAFC eligibility in Massachusetts?

Yes. Because GAFC is funded through MassHealth, financial eligibility applies. Income and asset limits are set by MassHealth and reviewed annually. If you are unsure whether you qualify financially, we can help you assess your situation before starting the application process.

Can GAFC be combined with other MassHealth programs?

Does GAFC cover room and board?

No. GAFC covers the cost of personal care services delivered within the housing setting. Room and board is a separate expense paid by the member.

Can a family member be a GAFC caregiver?

In some cases, yes. A trained family member may serve as a direct care aide under GAFC and be compensated hourly through the licensed provider. However, this is different from AFC, where a live-in family caregiver receives a monthly stipend.

Do I need to already have MassHealth to apply?

Yes, active MassHealth enrollment is required. If you are not yet covered, we help with that as part of our enrollment process.
