Group Adult Foster Care in Massachusetts is a MassHealth program that connects adults who need daily assistance with trained caregivers to receive personal care and support at home. It supports adults 22 and older, including people living with physical disabilities, cognitive impairments, or chronic conditions that affect their ability to manage daily life independently.
The program works through three connected pieces. MassHealth funds it. Licensed GAFC providers coordinate and deliver the care. And members can receive care at home through MassHealth-approved support.

What Services Does GAFC Cover?
GAFC covers two categories of daily support. A trained direct care aide provides one hour of hands-on support each day. The program also includes nursing oversight and case management, with a registered nurse and care manager conducting regular visits to review each member’s care plan, monitor health, and adjust support as needs evolve.

To qualify for Group Adult Foster Care in Massachusetts, a person must meet the following criteria. Clinical eligibility is not self-reported — a licensed clinician completes a functional needs assessment to determine whether the level of need meets MassHealth’s criteria.
Eligibility Requirements
- Be 22 years of age or older and a Massachusetts resident
- Have a documented clinical need for assistance with at least one ADL, confirmed by a physician
- Be enrolled in MassHealth and approved for GAFC services

A licensed clinician completes a functional needs assessment to confirm eligibility based on ADL need. Primary care physician approval is also required. We will contact the physician for approval.

Admission and Start of Care
Once approved, a registered nurse and case manager conduct an admission visit, your care plan is established, and daily services begin.
